Who will I be meeting with at the Diabetes Center?
Our outpatient diabetes program is led by experts trained in diabetes care. Through our comprehensive program you will meet with clinical nurse specialists, nurse practitioners, nurse educators, and dietitians who specialize in the management of diabetes.

Will I need to be seen again by my primary health care physician once I’ve been seen in the Diabetes Center?
Yes, your primary health care physician may want to see you back in three to six months. The Diabetes Center will keep your primary health care physician informed on how we have been working with you.

Will the Diabetes Center staff make referrals for additional services?
Yes, referrals for services such as exercise physiology and counseling services can be facilitated by Diabetes Center staff.

  • Counseling Services — Some patients may need additional emotional support in order to give their diabetes the attention it needs. This may help with keeping their life in balance with both physical and emotional health.
  • Endocrinology — patients may benefit from meeting a physician specializing in diabetes disorders for advanced treatment planning.
  • Podiatry — Patients may benefit from additional assessment and services to assist with evaluation of diabetes related foot disorders.
  • Opthalmology/Optometry — Diabetes can affect the eyes. It is recommended that patients with diabetes have dilated eye examination yearly. Establishing care with eye specialist is an important part of caring for your diabetes.
